Gingival recession defects are often first noted because they compromise aesthetics or dentinal hypersensitivity. Previously, procedures aimed to treat functional soft tissue defects were referred to as mucogingival surgeries. Periodontal plastic surgery is an approach that includes regenerative and reconstructive procedures designed not only to restore form and function but also to enhance aesthetics. One of the major goals of periodontal plastic surgery is to treat gingival recession defects through an array of classic and novel approaches. Another major objective is to change the periodontal phenotype from thin to thick in an effort to reduce the risk of future progression. This course will review the risk factors that influence disease progression and predict therapeutic outcomes. The rationale, scientific evidence and material selection for various therapeutic approaches will be discussed. In particular, vestibular incision subperiosteal tunnel access (VISTA) will be introduced as a predictable technique for periodontal and peri-implant soft tissue regeneration.
